Qualifications

Responsibilities

• Identify education needs of diabetes patients and communities.
• Deliver and evaluate patient education across illness lifespans.
• Lead outpatient diabetes and nutrition education program planning.
• Collaborate with primary care and multi-disciplinary teams.
• Participate in performance improvement to ensure quality care.
