Job summary

Wheeling Hospital, Inc. seeks a Diabetes Education Specialist to develop, manage, and coordinate all aspects of the outpatient diabetes program, including outreach to physician practices as referral sources for the diabetes program.

The candidate will provide education, coordinate care, and work with a multi-disciplinary team to improve patient outcomes, requiring CDCES certification and at least 1 year of health/fitness experience.

Qualifications

• CDCES certification required.
• 1 year experience in health and fitness.
• Ability to educate and coordinate diabetes care and outreach.

Responsibilities

• Develop, manage, and coordinate all aspects of the outpatient diabetes program.
• Outreach to physician practices as referral sources for the program.
• Perform patient education and assess goal attainment.
• Maintain ADA certification where applicable.
• Participate in community outreach and educational support.
• Collaborate with primary care providers and multidisciplinary team.
