Job summary

Linn County Health is seeking a DD Service Coordinator 1 to provide service coordination for individuals with developmental disabilities, including intake, needs assessment, and case management through the development of individualized service plans.

The role requires conducting needs assessments, writing clear reports, and maintaining professional relationships with staff, consumers, and providers. On-site work is required, with no remote option.

Qualifications

• Requires knowledge of case management and public service systems.
• Ability to conduct needs assessment interviews and develop individual support plans.
• Familiar with laws and administrative rules governing DD services and how they apply to service delivery.
• Strong report writing and client summaries; build good working relationships.

Responsibilities

• Provide intake, needs assessment and documentation for individuals with developmental disabilities.
• Develop and implement individualized service plans.
• Participate in multidisciplinary interagency meetings representing the Health Department.
• Provide information about available services to consumers and the public; perform case management.
• Maintain regular, predictable attendance and collegial work relations; train/orient new staff as needed.
