Job summary

Linn County's Developmental Disabilities Department seeks a DD Service Coordinator 1 to manage intake, assessments, and service planning for individuals with developmental disabilities. You will coordinate with multiple agencies, prepare plans, and document services in electronic records.

Qualifications

• Bachelor's degree in behavioral or social science or closely related field, or a 4-year degree with relevant experience.
• 1 year human services experience if Bachelor’s degree in any field; or 2 years if Associate degree.
• Equivalent combination of experience, education and training may be accepted.

Responsibilities

• Provide intake and needs assessment for individuals with developmental disabilities.
• Develop and monitor individualized service plans.
• Facilitate interagency meetings and coordinate with providers.
• Provide information to consumers and the public about available services.
• Maintain regular attendance and professional conduct; support case management and documentation.
