Job summary

The State of Maryland is seeking a full-time Developmental Disabilities (DD) Targeted Case Management (TCM) Coordinator of Community Services (CCS) in Cambridge, MD. This role provides a fixed entry point into the service delivery system for individuals with developmental disabilities and supports eligibility determination and PCP development.

Qualifications

• Education: Graduation from an accredited high school or possession of a high school equivalency certificate.
• Experience: One year of administrative staff or professional work in a health care setting.

Responsibilities

• Provide a fixed entry point into the service delivery system for individuals with developmental disabilities.
• Assist individuals in preparing applications and gathering documentation for DDA eligibility.
• Facilitate development of the Person-Centered Plan (PCP) and monitor PCP services.
• Complete additional assessments as determined by the client’s situation.
