Job summary

Total Care Services, Inc. is hiring a Coordinator of Community Services (CCS) to support individuals with IDD/DD in accessing services, resources, and community-based support. This is a full-time opportunity in Maryland.

The role welcomes case managers, social workers, and human services professionals who are passionate about serving the IDD/DD population and growing within a mission-driven organization.

Qualifications

• Bachelor’s degree in Human Services or a related field is required.
• 2+ years of case management experience or IDD/DD work.
• Must live in Maryland.
• Reliable transportation, valid driver’s license, and active car insurance.

Responsibilities

• Support individuals with IDD/DD in accessing services and community resources.
• Coordinate services and resources to promote independence and community participation.
• Assist with referrals, intake processes, and resource navigation.
