Qualifications

• Bachelor’s degree in Human Services or related field and 2 years working with persons with disabilities.
• Associate’s degree and 4 years of related experience with persons with disabilities.
• Must be at least 21 years old.
• Valid Pennsylvania Driver’s License with acceptable driving record.

Responsibilities

• Coordinate care and long-term program elements.
• Develop Individual Service Plans with clients, families, funding agency representative, and team members.
• Facilitate ISP/ITP meetings and Interdisciplinary Team Meetings with funding agency representatives.
• Serve as contact for monitoring visits.
• Advocate for the safety and welfare of clients.
• Coordinate admissions and discharge processes with Program Coordinator and Residential Manager.
• Maintain client files and timely documentation.
