Job summary

Beacon Specialized Living in Scranton, PA, is seeking a Case Manager (non-exempt) to coordinate care for individuals with developmental disabilities. You will develop service plans, facilitate interdisciplinary meetings, and ensure timely documentation while advocating for client safety.

This role requires a 21+ age, relevant degrees, driving, and strong communication. You will work with families, agencies, and treatment teams to sustain quality of care and community integration.

Qualifications

• Bachelor’s degree in Human Services or related field with two years of disability-service experience.
• Associate’s degree with four years of disability-service experience.
• Must be at least 21 years old.
• Valid Pennsylvania Driver's License with acceptable driving record.

Responsibilities

• Coordinate care and long-term program elements.
• Develop Individual Service Plans with client, family, funding agency representative, and treatment team.
• Facilitate plan meetings and documentation including progress notes and updates.
• Designated as contact for monitoring visits.
• Coordinate services among providers to ensure continuity of care.
• Attend meetings and transport clients to appointments.
