Job summary

CareWell Health is seeking a Clinical Care Coordinator RN to coordinate safe, patient-centered detox care, performing comprehensive nursing assessments and coordinating the plan of care. The role involves monitoring withdrawal symptoms, collaborating with physicians and behavioral health professionals, and guiding discharge planning.

The position requires active NJ RN licensure, ADN/BSN, and BLS/ACLS, with experience in acute-care or behavioral health settings.

Qualifications

• Active, unrestricted New Jersey RN license.
• Associate Degree in Nursing or BSN; BSN may be preferred.
• BLS/ACLS certification required.

Responsibilities

• Coordinate and facilitate safe, patient-centered care for withdrawal patients.
• Monitor withdrawal symptoms using CIWA, COWS & RASS.
• Administer medications per orders and monitor effects.
• Develop and evaluate individualized nursing care plans.
• Coordinate care among physicians, nurses, counselors, and social workers.
• Participate in treatment-team meetings and discharge planning.
