Job summary

CareWell Health Medical Center is seeking a Clinical Care Coordinator RN to coordinate safe, patient-centered care for individuals undergoing medically supervised withdrawal from alcohol and/or other substances. The RN will perform comprehensive nursing assessments, coordinate the plan of care, monitor withdrawal symptoms, and facilitate discharge and transition to the next level of treatment.

Qualifications

• Active, unrestricted New Jersey RN license.
• Associate degree in nursing; BSN may be preferred.
• BLS/ACLS certification required.

Responsibilities

• Perform comprehensive nursing assessments for detoxification and withdrawal management.
• Coordinate patient care plans and discharge planning.
• Monitor withdrawal symptoms and communicate changes to the care team.
• Collaborate with physicians, behavioral-health professionals, and other care team members.
• Maintain accurate documentation in the EMR and ensure regulatory compliance.
