Job summary

Mental Health Cooperative seeks a Discharge Planner for Detention Services in Nashville. This role coordinates discharge for patients in Davidson County Sheriff’s Office facilities, ensuring seamless transitions to community programs and continuity of care after release.

You will liaise with jail and healthcare teams, advocate for patient needs, and ensure policy-compliant documentation. The schedule is Monday–Friday, 8am–4:30pm (37.5 hours/week).

Qualifications

• Bachelor’s degree in a healthcare related field with experience in mental health.
• Case Management experience required or preferred.
• Experience in correctional healthcare is a plus.
• Strong communication, collaboration, initiative, organizational, and clinical skills.

Responsibilities

• Provide discharge planning for inmates in Davidson County Sheriff’s Office facilities and arrange community resources.
• Assess financial resources, make referrals, and assist with housing plans as needed.
• Work as part of a multidisciplinary team to address chronic/serious health conditions.
• Develop relationships with community resources to improve health outcomes.
• Document all discharge activities in the patient health record per policy.
• Coordinate with pre-release planning and healthcare providers for complex cases.
• Educate patients on continuing care and community referrals.
• Coordinate with medical staff to ensure prescribed medications are provided.
