Job summary

Qualifications

• Graduate from an accredited School of Social Work, Case Management, Health Care Management or other related fields.
• Two years of case management or related experience preferred.

Responsibilities

• Coordinate discharge planning for Chronic Care and other high-need patients.
• Engage community-based organizations to support discharge and transition.
• Collaborate with nurses, social workers, case management, parole/probation and other teams.
