Qualifications

• Bachelor’s degree in a healthcare related field.
• Case Management experience.
• Preference for experience in correctional healthcare settings.
• Excellent communication, collaboration, initiative, organizational and clinical skills.

Responsibilities

• Coordinate discharge planning for patients incarcerated in Davidson County facilities.
• Assess resources and arrange community referrals and housing as needed.
• Collaborate with multidisciplinary teams including jail staff and healthcare providers.
• Document discharge planning activities in the health record per policies and HIPAA.
• Provide patients with education on continuing care resources and community options.
