Qualifications

• Experience in discharge planning or case management is advantageous.

• Facilitates and organizes safe and timely discharge of patients requiring care coordination and resources.
• Advocates for patient and family to referral sources.
• Identifies and maintains information on agencies and resources for continuing care.
• Maintains accurate records and participates in process improvement initiatives.
• Supports the treatment team in locating resources for safe discharge.
