Job summary

DotHouse Health in Boston seeks a Deputy Director of Community Partnerships and Resource Navigation to lead patient navigation and community resource initiatives. You will work with clinical leadership to embed navigation into care delivery, improve workflows, and expand partnerships to address health-related social needs.

The role requires a Master's in a relevant field, 3+ years in navigation programs, and 2+ years in supervision.

Qualifications

• Master's degree in a relevant field is required (MSW/MPH/MHA/NP/NP-ic etc.).
• At least 3 years leading patient navigation, case management, or related programs.
• At least 2 years of supervisory or leadership experience.
• Experience developing community partnerships and collaborative initiatives.
• Grant-funded program management experience preferred.
• Experience in Federally Qualified Health Centers or value-based healthcare preferred.

Responsibilities

• Provide strategic leadership and operational oversight for community resource and patient navigation programs.
• Supervise navigation staff and develop staffing models, workflows, and quality standards.
• Develop partnerships with community organizations to strengthen resource availability and referrals.
• Lead data collection, reporting for grants, and quality improvement initiatives.
• Collaborate with clinical leaders to integrate navigation across care settings.
• Represent DotHouse Health in external committees and partnerships.
