Qualifications

• High school diploma or GED plus related experience in direct care with older adults living with dementia.
• Knowledge of dementia and ability to follow care plans and medication procedures where applicable.
• State certification may be required to assist with medications.

Responsibilities

• Provide dementia care, assist with activities of daily living, and support independence.
• Follow person-centered care plans and maintain resident dignity and engagement.
• Assist with meals, monitoring intake, and documenting changes in resident condition.
• Maintain accurate daily records and report health and safety concerns promptly.
• Collaborate with team and participate in care plan meetings as needed.
