PowerBack Rehabilitation in Milford, DE is seeking a Dementia Program Director to oversee the Memory Care unit. You will lead the Dementia Program, coordinate activities, and work within an interdisciplinary team to ensure high-quality resident care and engagement.

The role requires a health care background with dementia experience, supervisory experience, and eligibility for licensure. You will manage admissions/discharges and monitor the unit budget while advocating for residents and their

Qualifications

• Bachelor's or Master's in social work, nursing, therapeutic recreation, psychology, gerontology, or related field.
• Minimum of 3 years direct dementia care experience.
• At least 2 years supervisory experience in healthcare.
• State licensure in respective discipline recommended.
• Must meet state requirements for concurrent responsibilities.

Responsibilities

• Lead Dementia Program and coordinate patient/resident activities.
• Participate in interdisciplinary team meetings to ensure quality care.
• Assess patients and develop programming to maximize satisfaction and self-esteem.
• Perform quality improvement evaluations and report issues to the team.
• Create daily and monthly activity calendars for the unit and implement them.
• Advocate for patients/families and educate/support families.
