Qualifications

• Bachelor's degree in health care, mental health, or social services or licensed Health Facility Administrator (HFA).
• Minimum of one year dementia/Alzheimer's residents experience within the past five years.
• Three years' dementia care experience in healthcare, senior living, or long-term care.
• CDP credential preferred.

Responsibilities

• Direct and oversee the general daily operations of the Legacy neighborhood utilizing industry dementia best practices.
• Educate family members with the Family Orientation Guide and lead resident meetings and care plans.
• Maintain a dementia-safe environment through rooms, common areas, and security rounds.
• As Dementia Champion, provide dementia education to families and the community.
• Oversee Level of Care completion and follow-up per policy.
• Complete Social Services and Life Enrichment assessments and documentation for Legacy residents.
• Market and promote the Legacy Neighborhood to referral sources and dementia groups to achieve occupancy goals.
• Represent the Legacy Neighborhood at meetings for events, activities, and meal planning.
