Responsibilities
  • Provide appropriate supervision and training to persons served to facilitate their social wellbeing, health and safety in the least restrictive environment.
  • Administer medication to persons served according to medication administration procedures and upon completion of required state certifications.
  • Assist persons served in using public transportation or arranging transport to access community resources.
  • Plan and implement daily activities that promote social inclusion and build social capital for each person served.
  • Follow prescribed SJCS procedures to document supports provided, including progress toward skill acquisition, choices, health matters, achievements and challenges.
  • Attend and participate in all mandated events, workshops, training, investigations and meetings.
  • Ensure responsible usage of personal and agency resources such as vehicles, property and finances.
  • Perform other job‑related duties as assigned.
Minimum Experience, Education, and Requirements
  • High school diploma or G.E.D. required.
  • One or more years of experience working with persons with disabilities; experience in community‑based programs preferred.
  • Valid health certificate required to meet current funding jurisdiction requirements.
  • Valid state‑issued driver’s license with current driving record in good standing preferred.
  • Must meet SJCS and funding jurisdiction current requirements for criminal background/records checks.
Preferred Skills and Abilities
  • Knowledge of community resources that facilitate social inclusion and autonomy of persons served.
  • Ability to acquire working knowledge of St. John’s Community Services policies and procedures.
  • Ability to represent St. John’s Community Services publicly.
  • Effective communication and coordination skills with staff and individuals from diverse backgrounds.
  • Knowledge of acceptable meal assistance and safety practices and procedures.
  • Proficiency with relevant computer systems, hardware, software and office machines.
Physical and Environmental Conditions
  • Must be able to lift up to 30 pounds at times.
  • Work environment involves risks such as safe operation of a motor vehicle, lifting, transferring and/or positioning persons served, exposure to aggressive behavior and use of office equipment, and prevention of trips and falls.
