Job summary

Oaks Integrated Care is seeking a DDMI Housing Counselor to work in a supportive housing setting in Lumberton, NC. You will provide hands-on support in the community to help individuals maintain stability and access resources.

The role emphasizes outreach, enrollment, and coordination with a multidisciplinary team to promote client independence. The position offers a full-time schedule (Tue-Fri 9:00am–5:30pm with one night per week 11:00am–7:30pm) and a pay rate of $22.20 per hour, plus benefits

Qualifications

  • Bachelor's degree in a social service field is required or preferred.
  • Experience in the mental health field is preferred.
  • Valid driver’s license required and pre-employment drug screening may apply.

Responsibilities

  • Market the program and attend local and state meetings to promote services.
  • Monitor service levels and progress toward annual goals.
  • Outreach, engagement and assessment to enroll consumers in program services.
  • Maintain client case records and coordinate with the team for updates and treatment plans.
  • Assist clients in finding, selecting and furnishing affordable housing.
  • Help residents understand lease rights and manage disputes with landlords/other residents.
  • Assess client strengths/skill deficits for independent living and provide needed skill-building.
  • Connect clients with neighborhood resources (banks, transportation, stores, etc.).
  • Provide supportive counseling, problem solving, and crisis intervention as needed.
  • Collaborate with families and providers to develop service plans and transport clients for public transit.

Education

Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice or other "Helping Profession"

As a DDMI Housing Counselor, you will work in a supportive housing setting in Lumberton, Burlington County, NJ.

Bring care directly to those who need it most. Your work happens in the community, providing hands-on support in real-life settings to help individuals maintain stability, independence, and connection to resources.

Schedule: Full-time; Tuesday-Friday 9:00am to 5:30pm with one night per week 11:00am- 7:30pm

Pay: $22.20/hr.

Responsibilities:
  • Market the services of the program by attending local and State meetings
  • Monitor the Level of Service needed to meet the annual goals
  • Perform outreach, engagement and assessment to enroll consumers in program services
  • Maintain knowledge of each consumer and share knowledge with other team members through active participation in team meetings, regular review of consumer’s case record to ensure optimal responsiveness by staff to client’s changing needs, consult with other staff regarding treatment plan, and regular progress updates to other staff concerning work performed on consumer’s behalf
  • Assist client in finding, selecting and furnishing affordable housing of their choice and complete all necessary subsidiary documentation; ensure documentation is provided to DMHAS and other funding sources
  • Assist residents in understanding their rights and responsibilities under a tenant lease arrangement and mange disputes with other residents or landlords
  • Assess with clients strengths and skill deficits related to independent living; assisting clients in acquiring needed skills before and after placement in their new homes
  • Assist and link clients in learning about their neighborhoods (banks, stores, transportation, recreational opportunities and other community supports)
  • Provide supportive counseling and assist consumers in problem solving
  • Collaborate with consumers, family members and other providers to develop service plans
  • Transport consumers and teach them how to take public transportation
  • Provide services including but not limited to psycho-education and support, symptom assessment and management, family/significant other education and support, consumer advocacy and activities of daily living

Qualifications:
  • Bachelor's degree in a Social Work, Psychology, Sociology, Criminal Justice or other "Helping Profession";

  • Valid Driver's License required.
