The PATH program aims to connect homeless individuals with serious mental illness to housing and formal treatment services, with responsibilities including outreach, service coordination, and ongoing client support.

Qualifications

  • High School Diploma or equivalency and 2 years related experience, or Bachelor’s degree.
  • Valid Driver's License in good standing required.

Responsibilities

  • Provide outreach to shelters, train station, other identified locations.
  • Provide intensive community-based services to seriously and persistently mentally ill consumers in the community who are experiencing homelessness.
  • Provide intensive community-based services to seriously and persistently mentally ill consumers in the community for up to 1 year following housing.
  • Facilitate service linkage in the mental health and non-mental health health system; coordinating and integrating services from multiple providers, housing.
  • Provide direct service support including transportation, coordinating treatment team meetings, filing applications for services, and advocacy.
  • Develop and maintain collaborative relationships with landlords, boarding home operators and other housing resources providers.
  • Maintain an electronic consumer clinical record that accurately reflects all services provided on a timely basis and meets agency standards.
  • Complete Daily Activities in the electronic clinical record (MyEvolv and HMIS).
  • Compile and maintain an inventory of temporary and long-term housing services and providers.
  • Drive and travel within County of Service (Camden) required.
  • Participate with staff meetings and other program activities.
  • Perform other duties as assigned.

As a Case Manger, you will work with a dults experiencing homelessness and mental health needs located in Cherry Hill, Camden County NJ.

The Project for Assistance in Transition from Homelessness (PATH) program serves individuals in Camden County who are homeless or at risk of becoming homeless and who are not connected with any community services. The program places individuals with a serious mental illness in appropriate housing and links them to formal mental health treatment and systems to improve mental health functioning.

Pay:

$22.20/hr.

The employee will have the ability to assess and interpret information related to the population; have knowledge of psychiatric disorders and related symptoms and ability to recognize exacerbation of symptoms; have a general understanding of treatment needs; demonstrates knowledge of available community resources, including treatment programs and homeless services; population-specific communication skills

Responsibilities:
  • Provide outreach to shelters, train station, other identified locations.

  • Provide intensive community-based services to seriously and persistently mentally ill consumers in the community for up to 1 year following housing.
  • Facilitate service linkage in the mental health and non-mental health, health system; coordinating and integrating services from multiple providers, housing.
  • Provide direct service support including; transportation, coordinating treatment team meeting; filing applications for services; aggressive advocacy; escort to appointment; daily living skill remediation and money management.
  • Develop and maintain collaborative relationships with landlords, boarding home operators and other providers of housing resources.
  • Maintain an electronic consumer clinical record that accurately reflects all services provided on a timely basis and meets Agency and funding source standards.
  • Complete Daily Activities in the electronic clinical record (MyEvolv and HMIS).
  • Compile and maintain an inventory of temporary and long-term housing services and providers.
  • Drive and travel within County of Service (Camden) required.
  • Participate with staff meeting and other program meetings/activities.
  • Perform other duties as assigned.
