As a Case Manger, you will work with a dults experiencing homelessness and mental health needs located in Cherry Hill, Camden County NJ.
The Project for Assistance in Transition from Homelessness (PATH) program serves individuals in Camden County who are homeless or at risk of becoming homeless and who are not connected with any community services. The program places individuals with a serious mental illness in appropriate housing and links them to formal mental health treatment and systems to improve mental health functioning.
$22.20/hr.
The employee will have the ability to assess and interpret information related to the population; have knowledge of psychiatric disorders and related symptoms and ability to recognize exacerbation of symptoms; have a general understanding of treatment needs; demonstrates knowledge of available community resources, including treatment programs and homeless services; population-specific communication skills
