Job summary

Advocate Health in Charlotte, NC seeks a Registered Nurse to join the 36-hour-per-week, dayshift team (7a-7p) in the Care Coordination Center. The RN will apply the nursing process, engage patients and families, ensure safety, and collaborate across care teams to optimize outcomes.

Responsibilities include monitoring patient conditions, documenting in the EHR, and participating in quality initiatives. Requires BSN preferred, RN license with BLS, and typically 1 year of clinical nursing

Qualifications

• Graduate of a Board of Nursing approved nursing education program.
• Active RN license with privileges to practice in the state(s).
• Basic Life Support (BLS) certification required.

Responsibilities

• Engages in quality initiatives to improve care processes and apply evidence-based practices.
• Uses the nursing process to assess, plan, diagnose, implement, and evaluate care.
• Monitors patient conditions, adjusts care plans, and collaborates with the team to improve outcomes.
• Upholds a culture of safety and ensures accurate EHR documentation.
• May administer medications and therapies per protocols.
• Communicates effectively and delegates appropriately to team members.
