Qualifications

• Active LPN or RN license.
• Minimum 1 year LTC, SNF, Rehab experience required.
• Strong leadership, communication, and clinical skills.

Responsibilities

• Deliver and oversee resident care in accordance with all regulations and facility standards.
• Administer medications and treatments as prescribed.
• Supervise and support STNA team members.
• Communicate with physicians and families regarding resident status and changes.
• Monitor resident conditions and respond to clinical needs.
• Maintain accurate documentation and charting.
• Ensure a safe, clean, and compliant care environment.
• Assist with staff orientation and training.
