Responsibilities

• Assess patient needs across physical, psychological, social, and educational dimensions.
• Develop and revise plans of care with the multidisciplinary team to meet patient goals.
• Implement care per physician orders and nursing plan, ensuring safety and quality standards.
• Document assessments, plans, interventions and patient responses accurately in the medical record.
• Educate patients and families and provide discharge planning as part of continuity of care.
