Job summary

PACS Group is seeking an LPN/LVN for the Westerville facility to provide direct nursing care and supervise nursing assistants. The role involves coordinating daily nursing activities, ensuring compliance with regulations, and participating in care planning and discharge procedures.

Ideal candidates will have an active LPN/LVN license, nursing degree or equivalent, and strong leadership and communication skills to support residents and staff in a long-term care setting.

Qualifications

• Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
• Ability to supervise nursing assistants and coordinate care with nursing staff.
• Strong communication skills and ability to interact with residents, families, and staff.

Responsibilities

• Direct the day-to-day nursing care and supervise nursing assistants in accordance with regulations.
• Ensure adherence to facility policies, procedures, and quality assurance programs.
• Plan shifts, review manuals, and participate in care plan development and updates.
• Admit, transfer, and discharge residents and document care consistently.
• Maintain accurate charting, reports, and compliance with physician orders.
