Job summary

Sunflower Communities | Elmore Assisted Living in Elmore, Minnesota is seeking a Part-Time LPN for the day shift. You will deliver nursing care to residents in an Assisted Living and Memory Care setting, building relationships with residents and families.

Active Minnesota LPN license is required. Experience in long-term care or geriatric settings is preferred, with responsibilities including medication administration, monitoring changes, documentation, and coordinating with healthcare partners.

Qualifications

• Active Minnesota LPN license required.
• Experience in Assisted Living / Memory Care / Long-Term Care / Skilled Nursing / Rehabilitation / Home Health / Hospice or Geriatric Care preferred.

Responsibilities

• Administer medications and treatments according to provider orders.
• Monitor residents and respond to changes in condition.
• Provide nursing support for chronic conditions and cognitive impairment.
• Maintain accurate clinical documentation and medication records.
• Coordinate with physicians, pharmacies, home health, hospice, therapy, and other healthcare partners.
• Communicate changes and concerns with nursing leadership and providers.
• Participate in shared rotating on-call responsibilities.
