Job summary

Sunflower Communities | Elmore Assisted Living is seeking a Part-Time Licensed Practical Nurse (LPN) for the day shift in Elmore, MN. This role offers every-other-weekend coverage with rotating on-call and a $4,000 sign-on bonus, plus a competitive hourly rate of $30.00–$40.00 based on experience.

In this position you will administer medications, monitor residents, and support individualized resident plans, while building relationships with residents and families in a memory care and assisted

Responsibilities

• Communicate changes and concerns with nursing leadership and providers.
• Provide clinical guidance to caregivers and medication staff within scope.
• Coordinate with physicians, pharmacies, home health, hospice, therapy, and other partners.
• Participate in shared rotating on-call responsibilities.

As an LPN, you'll:
