Sunflower Communities | Elmore Assisted Living is seeking a Part-Time Licensed Practical Nurse (LPN) for the day shift in Elmore, MN. You will administer medications, monitor residents, and support care plans in a warm memory care environment.

This role offers meaningful clinical work and a stable community for residents and families. The position includes every other weekend and shared rotating on-call responsibilities, with a $30.00 - $40.00/hour compensation and a $4,000 sign-on bonus.

Qualifications

• Licensed Practical Nurse (LPN) in Minnesota.
• Experience with elderly residents and memory care preferred.
• Able to work part-time day shift and rotating on-call.

Responsibilities

• Administer medications and treatments per provider orders.
• Monitor residents and respond to changes in condition.
• Provide nursing support for chronic conditions and cognitive impairment.
• Maintain accurate clinical documentation and medication records.
• Support individualized resident service plans.
• Communicate changes with nursing leadership and providers.
• Coordinate with physicians, pharmacies, home health, hospice, therapy and other healthcare partners.
• Participate in shared rotating on-call responsibilities.

As an LPN, you'll:

• Administer medications and treatments according to provider orders
• Monitor residents and respond to changes in condition
