Responsibilities:
Provide direct nursing care to residents in accordance with care plans
Administer medications and treatments as prescribed
Monitor residents’ conditions and report changes to the RN and care team
Collaborate with CNAs, RNs, and interdisciplinary staff
Ensure residents receive care with dignity, respect, and compassion
Maintain accurate and timely documentation
Follow all safety, infection control, and regulatory guidelines
Qualifications:
Current LPN license in the state of Kentucky
Strong clinical skills and attention to detail
Excellent communication and teamwork abilities
A genuine passion for resident care
Long-term care experience preferred, but not required
