Qualifications

• Bachelor's degree in Social Work, Human Services, or similar health related field required.
• NVCI within three months of hire required.
• Minimum of 1 year of relevant experience preferred.
• Good working knowledge of computers.
• Ability to care for Behavioral Health age specific populations.

Responsibilities

• Coordinate discharge planning with patient and support system.
• Collaborate with interdisciplinary treatment team to plan and evaluate care.
