Responsibilities

Support daily living tasks such as personal care, meal preparation, and household routines.
Promote community inclusion and engagement for individuals with disabilities.
Encourage choice and independence in daily activities.
Build respectful, person-centered relationships with individuals served.
Follow individualized support plans and document progress regularly.
Communicate with families and team members to ensure consistent supports.
Ensure documentation is accurate and timely; comply with policies and regulations.
