Job summary

Communitas, Inc. in Wakefield, MA is seeking a Day Program Registered Nurse (RN) to join our clinical team. The RN will provide healthcare services, collaborate with families and residential providers, and support individuals across Wakefield and Woburn sites on a rotating schedule.

You must hold a Massachusetts RN license; new grads welcome. The role offers 30 hours per week (9am-3pm), full-time status, no on-call, nights or weekends, and a pay rate of $39 per hour.

Qualifications

• Registered Nurse with MA license.
• Ability to communicate with individuals, families, physicians and case managers.
• Pass background and reference checks.

Responsibilities

• Monitor health and well‑being of individuals and promote wellness.
• Administer medications and treatments as prescribed, ensuring regulatory compliance.
• Communicate health information to families and residential providers.
• Maintain medication records and member medical files for program compliance.
• Provide direct support and assist with lifting and transfers as needed.
• Coordinate care and ensure timely follow‑ups with DDS/DPH/CARF/Medicaid.
