Job summary

Tempus Unlimited, Inc. is seeking a Registered Nurse (RN) to support the Personal Care Management Evaluation Department in a community-based role across Massachusetts within Worcester coverage area. Expect a flexible schedule with a 35-hour work week, Monday through Friday core hours 8:30am to 4:30pm, and travel for field visits.

Requires MA RN license, valid driver’s license, and reliable transportation. Two-day orientation in Stoughton is required.

Qualifications

• MA RN license in good standing
• 2 years direct care of elderly or disabled individuals
• Bachelor’s degree preferred
• Familiarity with MassHealth PCA Program
• Fluency in other languages
• Must be able to travel within the coverage area

Responsibilities

• Perform initial evaluations and annual reevaluations for PCA applications in accordance with MassHealth regulations 130 CMR 422.422(C).
• Submit completed evaluation within the required timeframe and respond to hours adjustment requests promptly.
• Provide input regarding PCA evaluations and adhere to PCA Program standards.
• Schedule reevaluation visits and maintain calendar accordingly.
• Document progress notes in the database in a timely manner.
• Communicate with PCA Eval Dept Assistants regarding no shows, no contacts, hospitalizations, terminations, and concerns.
• Submit monthly mileage sheets within the required timeframe.
• Collaborate with PCA Program team and attend monthly staff meetings.
• Report suspected fraud to PCM Management Team and report abuse/neglect to the appropriate agency.
• Represent the agency professionally and maintain cultural sensitivity.
