Job summary

Cpmcares is seeking a Day Support Specialist to provide individuals with opportunities to develop valued skills and relationships. You will demonstrate initiative and follow through with program responsibilities to maximize participation.

In this role you will teach, supervise and assist participants in activities, transport them to settings, maintain accurate records, and coordinate with the Program Coordinator to support ISP objectives and personalized plans.

Qualifications

• High School diploma or GED.
• Valid driver1s license with reliable transportation.
• Knowledge of DDS systems.
• Medication Certification through DDS, First Aid, CPR and PMT certifications must be acquired within six months of hire date and maintained throughout employment.

Responsibilities

• Engage individuals in daily program tasks to increase aptitude.
• Teach, assist and supervise activities relevant to program scope.
• Organize outings and offer multiple options.
• Assist with personal care while maintaining proper technique.
• Record data on individuals' plans and levels of need.
• Maintain ongoing case records of individuals needs and services.
• Document time, attendance, and behavioral data.
• Organize and file records of daily routines.
• Document ISP objectives, behavior data, participation and production.
• Notify supervisor of any behavioral or routine changes.
• Transport individuals safely to/from residential and community settings.
• Transport between activities and work sites.
• Maintain vehicle cleanliness and logs; report damages.
• Perform other duties as assigned by the Program Coordinator.
