Job summary

You will perform daily program tasks, teach and supervise activities, organize outings, assist with personal care, and maintain accurate records, including ISP objectives and attendance.

Qualifications

• High School diploma or GED (required).
• Valid driver’s license with reliable transportation.
• Knowledge of DDS systems.
• Medication Certification through DDS must be acquired within six months of hire date and maintained.

Responsibilities

• Engage individuals daily in activities to increase aptitude and participation.
• Teach, assist, and supervise activities/jobs within program scope.
• Organize outings and present multiple options.
• Assist individuals with personal care using proper technique.
• Provide data on individual plans and needs; maintain case records.
• Document ISP objectives, behavior data, participation, and production.
• Notify supervisor of behavioral or routine changes, and transport individuals safely to activities and sites.
• Maintain vehicle cleanliness and logs; perform other duties as assigned.
