Job summary

Kaizen Lab Inc. is seeking a Day Support Specialist to provide opportunities for individuals to develop and maintain valued skills and relationships.

The role requires initiative and reliable follow-through to maximize participation in program activities. Responsibilities include engaging individuals in daily tasks, assisting with personal care, organizing outings, and maintaining accurate records and transportation for daily activities.

Qualifications

• Knowledge of DDS systems required.
• Medication Certification via DDS must be acquired within six months of hire date.

Responsibilities

• Perform daily program tasks to actively engage individuals in activities and settings.
• Teach, assist, and supervise individuals in activities/jobs related to the program scope.
• Organize outings and present individuals with multiple options.
• Assist individuals with personal care while maintaining proper technique.
• Provide data and input on individual’s plan and level of need.
• Maintain ongoing case records of individuals’ needs and services.
• Document the individual’s time, attendance, and behavioral data.
• Organize and file records for daily routines.
• Document ISP objectives, behavior data, participation, and production for each individual.
• Notify supervisor of any behavioral or routine changes.
• Transport individuals safely to and from residential and community settings.
• Maintain vehicle cleanliness and accurate vehicle logs.
• Perform additional duties as assigned by the Program Coordinator.

Skills
