Job summary

Qualifications

Responsibilities

• Implements IPPs and assists consumers to meet goals; documents performance.
• Supervises consumers in tasks, ensuring safety and adherence to procedures.
• Oversees community-based activities and records progress.
• Maintains safe environment and reports hazards or maintenance needs.
• Completes required documentation and reports monthly summaries.
• Provides on-demand support to consumers, including training and self-care.
