Job summary

The role requires creating individualized plans, supervising staff, and ensuring Medicaid compliance, logging data, and safety. A related bachelor’s degree and 2 years of relevant experience are preferred; QIDP experience is desirable.

Qualifications

• Bachelor’s degree preferred; two years of experience with intellectually disabled and/or physically/multiply disabled required.
• Qualified Intellectual Disability Professional (QIDP) experience desirable.
• Ability to read and interpret safety rules, operating instructions, and manuals; document well.

Responsibilities

• Lead and coordinate activities for developmentally disabled individuals in Day Habilitation/Pre-vocational services.
• Develop and implement activities, curricula, and lessons; participate with staff activities.
• Formulate individualized service plans with assessments, progress notes, and data collection.
• Complete Medicaid documentation; ensure daily logs are maintained by DSPs.
• Maintain a clean, safe program area; ensure supplies and equipment are in good condition.
• Assist with transporting individuals to activities; drive as needed.
