Job summary

RTR is hiring for a Part Time Day Habilitation Specialist in Middleboro, MA. You will provide 1:1 and group support, help adults with developmental disabilities develop daily living skills, and implement care plans with an interdisciplinary team.

You’ll track progress, document clearly, assist with transportation, mobility and dietary support, and collaborate with families and community partners. Requires a High School diploma and driver’s license. Sign-on bonus applies.

Qualifications

• High School Diploma or equivalent.
• Valid Driver’s License.
• Basic knowledge of Microsoft Word and the internet.
• Successful completion of all state and agency mandated trainings.
• Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning related agencies.
• Must be able to perform each essential duty satisfactorily.

Responsibilities

• Provide 1:1 and group support based on individualized service plans.
• Help individuals develop skills in communication, self-care, and daily living.
• Implement care plans developed by clinical teams (OT, PT, Speech, Nursing, Behavioral).
• Track progress and complete clear, professional documentation.
• Support community engagement, including transportation and activities when needed.
• Assist with hands-on care needs, including mobility and dietary support.
• Collaborate with team members, families, and community partners.
• Serve as a positive role model, promoting safe and respectful environments.
