Job summary

Heartshare is seeking a Day Hab Support Coordinator to oversee Day Habilitation staff and ensure comprehensive coordination of participants’ needs and goals. You will supervise staff, monitor plans, and maintain documentation across Life Plans and program records.

The role requires commitment to regulatory compliance, timely reporting, and collaboration with care managers, family members, and external agencies to support participants in the community and program settings.

Qualifications

• High School Diploma required.
• Minimum one-year experience working with individuals with developmental disabilities.
• Valid NYS driver's license preferred but not required.

Responsibilities

• Participate in required meetings and ensure recommendations are implemented.
• Oversee coordination of participants' needs and goals with care teams.
• Maintain Life Plans, Staff Action Plans, and progress notes with timely updates.
• Certify and review staff action plans and ensure documentation is complete and accurate.
• Ensure program compliance with agency, state, and federal regulations and timely reporting.
