Job summary

Community Connections Inc. is seeking a Day Hab Aide in Mashpee, MA to support individuals with disabilities by implementing service plans and promoting independence. This non-exempt, hourly role emphasizes compassionate care and community support, with opportunities for growth in human services.

Qualifications

• Bi-lingual (Portuguese) is preferred.
• High School Diploma or GED required or preferred training/experience with individuals with disabilities.
• Experience and/or training working with individuals with disabilities preferred.

Responsibilities

• Assist individuals in developing and refining skills, promoting functional independence during breaks, mealtimes and other routines.
• Help to implement DHSP goals within a group setting or one-to-one.
• Assist individuals physically as necessary.
• Document individuals progress toward DHSP goals.
• Ensure individuals health, safety and general well-being.
• Safeguard the human dignity, rights and liberties of the individuals we serve.
