Job summary

Community Connections Inc. is seeking a daytime Aide for our Wareham Day Center to support individuals with disabilities by implementing service plans and promoting independence.

You will work with a team to assist with activities, monitor health and safety, and document progress toward individual goals. A high school diploma is required; bilingual Portuguese preferred and a valid driver’s license is a plus.

Qualifications

• High School Diploma or GED required.
• Experience and/or training working with individuals with disabilities preferred.
• Valid driver's license and satisfactory driving record preferred.
• Ability to lift 30lbs, assist individuals in/out of wheelchairs or up from/down to mats.
• Demonstrated ability to work independently.
• Bi-lingual preferred (Portuguese).

Responsibilities

• Assist individuals in developing and refining skills, promoting functional independence during breaks, mealtimes and other specific routines.
• Help to implement DHSP goals within a group setting or a one to one situation.
• Assist individuals physically as necessary.
• Document individuals progress toward DHSP goals.
• Ensure individuals health, safety and general well-being.
• Ensure and safeguard the human dignity, rights and liberties of the individuals we serve.
