The role involves delivering compassionate, hands-on care in a PACE Day Center, supporting frail elderly participants under supervision, and following individualized care plans to promote comfort and dignity.

The position requires Michigan CNA certification, a high school diploma or GED, and the ability to lift up to 35 pounds. Weekend/evening coverage and CPR within 3 months are noted as desirable.

Qualifications

• Must be certified as a Certified Nurse Aide in Michigan.
• High School Diploma or GED required.
• Ability to lift and move objects weighing up to 35 pounds without assistance.
• Must have reliable transportation.
• Flexibility to provide coverage in the evening and on weekends.
• Have one year experience working with the frail elderly population.
• Valid driver’s license.
• CPR certification is desired or obtained within 3 months of employment.

Responsibilities

• Provide Hands-On Care– Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan.
• Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.
• Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
• Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
• Provide Responsive Support – Respond with kindness and compassion.
• Monitor with Watchful Eyes – Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP.
• Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

Compensation: $18.57 - $20.70 / hour

Schedule: PartTime | 24 hours per week | 8 am - 4:30 pm | Monday - Friday

What You'll Do In This Role: Make a meaningful difference every day as a Certified Nurse Aide (CNA) in our PACE Day Center, providing compassionate, hands-on care and support to frail elderly participants. Working closely with the healthcare team, you will help carry out individualized care plans that promote comfort, dignity, independence, and overall well-being in a warm, participant-centered environment. Under the guidance of the Day Center Supervisor, this role offers the opportunity to build meaningful connections while making a lasting impact in the lives of older adults.
