Qualifications

• At least 3 months experience implementing behavioral support plans and emergency physical interventions.
• High school diploma or equivalent; associate or bachelor’s degree preferred.
• A minimum of one year experience working with individuals with brain injury, developmental and/or physical disabilities is required.
• Ability to record data using a computer, enter time worked into an electronic system and receive and send email is required.
• A valid driver's license in state of residence preferred.
• Ability to comprehend materials related to ISP’s/DHSP’s, PBS, ADL programs and individual therapeutic programs.
• Ability to speak and write English clearly and accurately to keep documentation up to date.
• Ability to communicate effectively, both verbally and in writing with persons served, families, service providers and funding source personnel.

Responsibilities

• Assist individuals served with personal care tasks including toileting, grooming, personal hygiene, mobility, repositioning, dressing and ROM.
• Support individuals in completing ADLs and Independent Living tasks such as cooking, cleaning, shopping, and money management.
• Utilize Positive Behavior Supports (PBS) to align activities with individual preferences and goals.
• Ensure nutritional needs are met according to prescribed diets; assist with feeding when necessary.
• Use and maintain health-related protections/equipment; clean equipment as directed.
• Learn and utilize communication tools (email, tablets, letter boards) to suit individual needs.
• Assist in planning and implementing community-based and day program activities; facilitate group activities.
• Teach and support financial and safety skills to promote independence.
• Assist in developing and implementing ISP/DHSP and behavioral plans.
• Maintain cleanliness of indoor/outdoor areas; transport individuals to activities as required.
• Adhere to approved restraint techniques only as necessary to prevent injury.
• Record data at shift end; document in notebooks, ISP/DHSP data, and behavior tracking.
