Job summary

Community Care, Inc. is seeking a Data Specialist to support data collection, reporting, and health records integrity within our Vliet Office.

The role collaborates with the Director of Medical Informatics and IT/Medical Informatics staff to ensure accurate data across Claims, EMR, and internal systems. You will prepare and submit required reports (DataPACE2, HPMS, CMS, DHS), monitor data processes, and provide admin support to the department.

Qualifications

• Three years of health care office experience with data entry and reporting.
• Associates degree or higher in related field; or 5 years equivalent experience.
• Proficiency in Microsoft applications and data reconciliation.

Responsibilities

• Data collection, recording and management for reports and databases.
• Assures integrity of health records and medical records through evaluation and auditing.
• Reviews health records for completeness and informs manager of deficiencies.
• Monitors key automated processes and escalates failures per protocol.
• Gather requirements and build/ deliver reports to internal, external, or regulatory bodies with SLAs.
