Qualifications

• Bachelor’s degree, or associate’s degree and three (3) years’ experience.
• Licensed as a Registered Nurse.
• Documented case management and/or administrative experience preferred.

Responsibilities

• Assess, plan, implement, coordinate, monitor and evaluate patient needs to promote quality outcomes.
• Manage high-risk patient registry and coordinate care across the continuum.
• Support acceptance of referrals, discharge planning and coordination with care teams.
