Resources for Human Development in Rapid City, SD is seeking a dedicated Case Manager to coordinate care for youth and families. You will work in a multidisciplinary team to develop service plans, monitor progress, and connect participants with community resources.

The role emphasizes collaboration, professional development, and hands-on experience in behavioral health and case management, with a strong focus on outcomes and adherence to policies and HIPAA regulations.

Competenze

• Bachelor’s degree or equivalent combination of education and experience.
• Intermediate computer skills and HIPAA compliance readiness.
• Must pass drug screening and background checks.

Mansioni

• Act as part of a multidisciplinary team to provide holistic services.
• Develop and maintain ISP in collaboration with participants and medical staff.
• Coordinate with community agencies to connect participants with resources.
• Monitor ISP, document results, and intervene as necessary.
• Process and review Critical Incident Reports per protocol.
• Participate in Human Rights and Behavioral Intervention Committee reviews.
• Focus on Personal Outcome measures and Life Course planning.
• Link participants to needed programs and supports.
• Complete required documentation in Therap.

What You'll Do

Act as part of a multidisciplinary team designed to provide services from a holistic and Values

based perspective.

• Assist in the development of service plans and reviews in collaboration with the program participant, medical team staff and other stakeholders as determined in collaboration with program participant.
• Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.
• Facilitate team process for the initial and annual development of the Individual Service Plan (ISP)
• Ongoing monitoring of ISP and documenting results and intervene as necessary
• Report and review Critical Incident Reports (CIR) per protocol
• Participate and monitor Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) reviews as appropriate
• Focus on Personal Outcome measures
• Knowledge and understanding of Charting the Life Course framework and Person Centered Thinking/Planning
• Evaluate the needs of those on caseload in order to remain successful in the community
• Link those on caseload to programs and supports that are needed and requested to meet their needs
• Complete required documentation of services, including input of data, reports, and plans into Therap.

QualificationsRequired

Bachelor’s degree and one year of experience preferred OR equivalent combination of education and experience.

• Intermediate computer skills
• Have the ability to establish a HIPAA compliant home office
• Valid driver’s license.
• Must pass drug screening and state and federal background checks

Preferred

• Previous experience in behavioral health, human services, case management, or residential care settings
• Experience working with adolescents and families

Schedule

• Full-Time
• Monday-Friday

Physical Requirements (If Applicable)

• Ability to lift up to 25 pounds
• Ability to stand, walk, bend, and assist youth as needed
• Ability to perform essential job functions with or without reasonable accommodation
