Job summary

Family Residences and Essential Enterprises, Inc. (FREE) seeks an Enhanced Step-Down CTI Case Manager to support adults with co-occurring SMI and I/DD transitioning from hospital settings into the community.

You will deliver CTI-based, trauma-informed case management and help connect clients to long-term supports. Work with hospital discharge planners, OPWDD, and community providers, document thoroughly, and participate in team meetings to maintain program fidelity.

Qualifications

• Must have experience working with individuals with co-occurring I/DD and SMI.
• Experience navigating OMH/OPWDD services and eligibility processes.
• Preferred certifications such as CTI and OMH 9.58 designation.

Responsibilities

• Provide specialty CTI services with fidelity to the model for individuals with co-occurring I/DD/SMI.
• Develop Immediate Needs Assessments and individualized transition plans across housing, health, and community supports.
• Collaborate with hospital discharge planners, OPWDD liaisons, and providers to ensure seamless transitions.
