Qualifications

• Master’s Degree in social work, psychology, rehab counseling, mental health counseling, nursing, or related field required.
• Experience with individuals with Intellectual/Developmental Disabilities, mental illness, and/or substance use disorders preferred.
• Complete CTI certification within 12 months; 9.58 designation within 6 months; LCSW/LMHC/LMSW or related licensure preferred.

Responsibilities

• Coordinate assigned Care Managers and Peer Specialist under CTI supervision.
• Provide time-limited, evidence-based case management to individuals during transitions.
• Link individuals to longer-term professional and community supports.
• Collaborate with community partners and internal teams to design trainings.
• Maintain documentation and data to meet required quality standards.
• Provide crisis intervention and 24/7 on-call coverage as needed.
