Job summary

The Bridge in New York, NY is seeking a Case Manager to support participants transitioning from street life to stable housing under the CTI model. You will conduct outreach, assess needs, accompany clients to appointments, and advocate for equitable access to services.

This role requires field work, flexible hours, and a commitment to harm-reduction and person-centered care, with training opportunities in CTI and professional development.

Qualifications

• Bachelor’s Degree; Minimum three years of experience providing services to adults with serious mental illness preferred.
• Experience with homeless or precariously housed populations preferred.
• Knowledge of NYC shelter systems and MTA transit systems a plus.
• Knowledge of counseling principles for mental illness and substance use disorders.
• Ability to prepare accurate and timely reports and maintain documentation.

Responsibilities

• Persistent outreach and engagement using strength-based approaches.
• Assess health and social needs via conversational and risk assessment tools.
• Identify housing options, assist with housing applications and move-in prep.
• Participate in discharge planning meetings and connect to community resources.
• Collect and report data; collaborate with team to inform care delivery.
• Support clients post-housing to resolve clinical issues and barriers to care.
